Every aspect of the graft is vital—that’s why you spend so much time, effort, and skill creating it. Each time you anastomose, it may be just another surgery to you, but it’s a miracle for the patient and their family.
So what if I could offer you a related miracle?
What if I—by which I mean you—could answer nearly every question you’ve ever had about the performance and function of each graft? Is the peak flow sufficient? What is the minimum flow? Does the peak flow taper in a healthy manner or drop off sharply because there’s downstream resistance that you need to know about? Is there competitive flow through the native artery that’s stealing oxygen from the tissue, even though the mean flow appears acceptable?
All of these questions and more are answered by years of calculations and complex programming, courtesy-distilled for you into the simple curvature of a red and blue line.
So this “miracle” to which I’m referring is waveform analysis. Transonic has engineered the fundamental solution to anastomosis questions via the continuous graphing of flow measurements.
If competitive flow exists, part of the cardiac cycle will fall below the zero line (retrograde flow) showing that backflow through the occluded vessel offers lower resistance than the downstream vessels. In this case, peak flow through the graft appears sufficient, but a portion of that blood is recirculating, reducing overall oxygenation to the target tissue.
And that barely skims the surface of waveform analysis.
For CABG in particular, Transonic has developed an exhaustive handbook, completed with hundreds of waveform images illustrating not only patent, sustainable grafts, but images of altered waveforms representing the variety of problems that can occur in CABG grafting.
Furthermore, the plethora of illustrations is broken down by graft type.
For example, if you’re performing a LIMA to LAD, but the waveform doesn’t look like the image of a perfect LIMA/LAD waveform, the Handbook offers a trove of images showing what the waveform will look like if there is a stitch through the backwall, or if the native is not fully occluded, or if there is competitive flow, etc, etc, etc. Not only do waveforms help you determine if a graft is patent, but if it is not, they help you determine—in crystal clear pictorial fashion—what may be the cause.
